When there is an instrument failure that needs troubleshooting during the afternoon or during the night, a night log reporting the problem needs to be filled in, indicating the time lost. This is of special importance during the night to keep track of the efficiency of the observations.
This is the webpage where you can find all logs ever filled, and where you can fill in your own logs: https://www.keck.hawaii.edu/software/ndlog/webForm/index.php. Be aware that there is a password to fill in a new log. Check it out in “showpasswords“ (ask some of the other SAs how that works).
Once you access, you will find a form similar to this one:

The following fields need to be filled in to make the log informative enough:
1/ Topic: describing the issue
2/ UT/HST time in which the problem began
3/ From: your name
4/ Urgency: reserve “Critical” and “High” for really urgent problems that need to taken care of immediately.
5/ Keyword: indicating the topic to which the problem is related, and the instrument.
6/ Resume work: indicating at which time the observations were resumed.
7/ Details: explained the problem in detail and how you solved it.
